*A parent or guardian must be enrolled in the Adult Block Fee Program for children and teens to join. Children under one are included. Add four additional psychotherapy sessions for $1,000/year. Final fees are confirmed at your Meet & Greet.

No. You may apply to continue with Dr. Kennedy at JW Health or arrange ongoing care elsewhere.
No. You must complete JW Health's enrolment process, sign a new agreement and pay the applicable fee.
Yes. Fees paid to Inliv do not transfer to JW Health and cannot be credited, prorated or applied to your JW Health enrolment.
No. Questions about Inliv payments, unused services, contractual obligations or possible refunds must be directed to Inliv.
Records are handled separately from enrolment. You may need to provide consent or complete a records-transfer authorization.
Before October 30, direct questions about active care to Inliv. After your care at JW Health begins, JW Health will explain how active items will be followed.
Inliv appointments do not transfer. Confirmed JW Health patients will receive new booking instructions.
Family members may apply, subject to eligibility, physician availability and practice capacity. Applicable fees will apply.
You will need to arrange ongoing care elsewhere and may contact Inliv about accessing or transferring your records.
